We are so excited that your child is going to be joining us for Summer Camp! Before you fill out the registration form please read the following:

This registration form must be filled out FOR EACH CHILD individually. In addition to filling out the registration form please return to faithchurchbrooksville.com/summercamps and download and fill out the necessary paperwork and Medical Histories for your child(ren).

PAYMENTS: Summer Camp Payments can be made at the bottom of this form for all of your children. One form per child per week.

CAMP COST: $125 per week Monday to Friday 8:00AM-4:00PM
